OVERVIEW & BENEFITS - CASE MANAGEMENT

The Case Management program supports individuals and their families facing difficult times in their lives. Our experienced and compassionate professional staff conducts interviews or, if appropriate, comprehensive in-home assessments to determine client needs. Our professional staff then links clients to appropriate services and provides follow-up and further coordination as needed.

The Case Management program also refers to social services accessed through the City of Albuquerque/Bernalillo County Department of Senior Affairs.

JFS is here to assist you if you are experiencing difficulty with:
• Aging-related issues
• Housing
• Navigating social systems (Medicare, Medicaid, Income Support Division)
• Obtaining increased levels of care

SERVICES

JFS case managers offer a wide scope of services, including:
• Advocacy
• Information and assistance
• Assessments
• Coordination of service with providers
• Family caregiver support
• Education
• Medical case management (medication set- up &
  management, medical advocacy)

Services JFS can refer to through the Department of Senior Affairs are: home chores/retrofit/repair, intensive case management, homemaker/respite, special needs transportation, in-home respite, and senior day care. Clients must be age 60 or older and reside in Bernalillo County to access these services.

Fees: Some services, including those available through the Department of Senior Affairs, are free. A sliding fee scale may apply to other services.

CLIENT SUCCESS STORY - "A Better Outlook on Life"

A JFS Case Manager conducted an assessment and made service referrals for a client. The client was having difficulty getting around in her home, dealing with depression and eating nutritiously. Referrals included an evaluation by a Physical Therapist, home delivered meals, homemaker services, the JFS Nurse Case Manager, and JFS counseling services. The client reported two months later, that after working with the different services being provided, she now feels like she is coming out of a “fog” and “enjoys life more and more each day.” She reported that she goes for walks every day, has changed her doctor, and feels like the services we referred her to “helped her get to a better place.”
